Psychological Issues
A number of treatments options are available to deal with the psychological issues related to impotence. Sometimes psychological issues stem from impotence and at other times they are the cause of impotence. These issues may range from relational difficulties to difficulties that have arisen from instances of failure to achieve an adequate erection, leading to performance anxiety. When sleep induced or self-stimulated erections are firmer than those achievable through having a sexual relationship with ones partner, it is important to look for psychological or relationship difficulties.
Vacuum Erection Devices (VED)
A vacuum erection device is a mechanical device consisting of a cylinder that is placed over the penis, and a pump attached to the cylinder, which when turned on creates a vacuum in the cylinder, causing blood to be drawn into the penis. When sufficient blood has flowed into the penis to provide an erection firm enough for intercourse, an elastic band is placed at the top of the penis to prevent the blood from flowing back out and the cylinder is removed. The primary complaint about this device is the discomfort caused by the degree of tightness required for the elastic band to be effective.
Surgical Implants
Two types of surgical implants exist, malleable and inflatable. Malleable implants are made of a semi-rigid core covered by an outer shell. These implants, though easy to put in, are less frequently used because the penis remains erect at all times, with an erection that does not appear all that normal.
A more commonly used type of implant, and often preferred above penile injections is the inflatable device. These devices generally contain a pair of cylinders inserted into the penis, a small pump in the scrotum and a reservoir filled with fluid to be pumped into the penis when needed. Over the years significant improvements have been made in these devises, resulting in increased satisfaction among recipients and their partners.
Vascular Surgery
Two types of vascular surgery are available depending on the nature of the problem. One involves penile artery bypass surgery to increase the blood flow into the arteries of the penis to increase the firmness of the erection. A second type of surgery, in the experimental stages which appears to be showing promise is venous ligation surgery, which is designed to repair to the venous blood vessels that are leaking fluid out of the penis, causing the erection to fail.
Summary
Impotence is a common problem reported by 39 to 67% of men over the age of 40. Some excellent treatments are available with improvements being added all the time. It is important for any couple suffering from difficulties with impotence to discuss this issue with their health care provider, where they can select the most appropriate and safe treatment for their needs.
